The safety of your child is our primary concern. Precautions will be taken for their well-being and protection. While every precaution is taken for the safety and good health, some games and activities carry with them the inherent risk of personal injury beyond the risks associated with many of the recreational activities at AWANA.
I/we understand and accept these risks and agree that by allowing my/our child to participate in those activities, he/she may be taking part in a recreational activity that presents the potential for personal injury.
I/we hereby consent to the participation of my/our child in C3 Church Calgary AWANA supervised activities.
I/we, the Parents or guardians named below, authorize C3 Church Calgary personnel to
- administer basic first aid medical treatment
- to call an ambulance in an emergency
- to authorize a hospital personnel to provide medical assessment/ triage and initial basic treatment for the participan(s) named above.
** The leaders will first and foremost contact the emergency contact provided above. In an emergency situation that involves EMS, they will do all that is in their job scope; by the time that a child arrives at the hospital, parents should have been contacted and likely be present to make further decisions.

We need your support and help to fulfill the vision of this ministry!Our desire for Awana at C3 Church is for it to be part of our larger vision of mid-week discipleship opportunities for the entire family. In order to have AWANA, there are registration/eligibility requirements. A parent is signed up to be a team member for Awana OR a parent is signed up for a mid-week growth group program running on Wednesday night at C3 Calgary West Team members are essential to the success of Awana. There are a lot of opportunities for you to be part of this exciting discipleship opportunity!

Awana registration fees for September 2024 - June 2025 at C3 Church Calgary will be $110.00 per child. Cost covers all material that will be needed for this year. Please specify AWANA and your family name.

I/we, named below, undertake and agree to indemnify and hold blameless C3 Church Calgary, its personnel, leaders, and Board from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of C3 Church Calgary AWANA, as well as of any medical treatment authorized by the supervising individuals representing C3 Church Calgary. This consent and authorization is effective only when participating in events of the C3 Church Calgary AWANA. C3 Church Calgary is collecting and retaining this personal information for the purpose of maintaining contact information should an emergency/injury arise during AWANA. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. This information is not shared with any other organization.
